[Bug] Model autonomously executes decisions without user confirmation in scoped requests

Bug Description
the model systematically overruns scoped requests / folds findings into bookkeeping. I had a couple situations where the model was asked to do some research and presents its findings for a collaborative decision and either the results were not showed, just stored into memory and a brief summary what was done was presented (without the actual data) or fable did a decision on its own (which i wanted to participate it) and changed something autonomously. Opus did not do this. I appreciate the autonomy but this is happening too often.
