[Bug] Manual approval of blocked action not persisting; [Credential Exploration] denial recurring on retry

Bug Description
In /permissions → Recently denied, I selected a blocked action and pressed Enter to approve. When retried, the action was denied again with [Credential Exploration].
The action was one explicitly authorized read-only aggregate query through Supabase’s documented /database/query/read-only endpoint. No credential was read and no request executed.
Please investigate why manual approval did not permit this action or explain the supported approval path.
