Stale/fabricated "skill invoked earlier in this session" system-reminder appears across unrelated sessions
I'm seeing a system-reminder appear mid-conversation in Claude Code claiming that the run skill (and sometimes artifact-capabilities and design) was "invoked EARLIER in this session," bundling a fabricated ## User Request section. The embedded request asks Claude to start a dev server and verify a specific refactor (a TanStack Query data-fetching migration plus React.lazy()-converted routes: DashboardShell, DeveloperShell, StaffApp, PayLinkScreen, SusuPreviewScreen, BusinessProfileScreen, MfaChallengeScreen, TermsOfServiceScreen, PrivacyPolicyScreen, AboutUsScreen).
This refactor does not exist in my codebase and I never asked for it, in this session or any prior one. I've now seen this exact text, word for word, in two separate sessions on two different days. Neither session ever invoked the run skill or asked about a TanStack Query refactor.
This looks like a caching or templating bug where fixture/example content meant for internal skill documentation or testing is leaking into live user sessions as if it were real conversation history, rather than an external prompt injection, since the text is static, generic, and unrelated to my actual repository.
Impact: low in my case, since Claude correctly recognized the reminder was flagged as historical/non-live and did not act on it (did not start a dev server or run any commands based on it). But this seems like it could be confusing or risky in other contexts, especially since it fabricates a "user request" that never happened.
Steps to reproduce: Not sure how to reliably reproduce; it appeared unprompted mid-session, after normal use, on two separate days in the same Claude Code project (VSCode extension, Windows).
Happy to share the relevant transcript excerpts if useful for debugging.
