[Bug] No notification when model automatically downgraded due to safeguards

Bug Description
I'm using Fable. Sometimes it hits safeguards for some unknown reason, and the model changes to Opus 4.8. However there is no notification at all, and the work proceeds without me knowing. Opus 4.8 isn't capable enough for the task I'm working on, so I end up wasting a lot of time using the wrong model. I understand you need the safeguards, but I should at least get a notification, and work should probably stop too. Also it drops to Opus 4.8, not Opus 5, which is very strange.
