[Opus 5] Doesn't follow the instruction at the user level

What You Asked Claude to Do

my user level instructions are very clear - "Do no repeat" "Keep response concise"
then i asked it to review the remediation plan, validate the issues and fix them, get it reviewed by an enterprise architect using Fable model, fix the reviews using Opus model and get all the issues closed

What Claude Actually Did

Didn't follow the instructions - was too chatty and repeating the same status more than 3 times
fixed a few bugs, but obediently came back and said 6 are left for my decisions.
