[Bug] Unintended Document Duplication Instead of Update

Bug Description
i asked it explicitly to update a document it already created, and it just made a new one. recently, it has been making copious amounts of markdown docs explaining itself. i lose confidence that it is not writing tight simple code if the documentation is sprawling and out of control.
