Bug Description
Claude was reading documents for me. This was a simple job, read the pdf, copy it to markdown. That's it. It read the first file ($filename_v1.pdf) and wrote it. It looked at the 2nd file ($filename_v2.pdf) and confidently declared it to be a revision of the first file, when it was actually two separate reviews of two different books (vol 1 & vol2). Claude had enough information to make this determination easily, but chose to be a lazy liar instead.
