Session not found on disk — all my sessions are affected

What's Wrong?

All of my sessions are showing "Session not found on disk" when I try to open them. Every conversation and project I had is affected, not just one or two — my entire session history appears inaccessible.

What Should Happen?

Previous sessions should open normally and show the full conversation history.

Steps to Reproduce

Open Claude Code and go to the sessions/recents list
Click on any previous session
Error message "Session not found on disk" appears instead of the conversation
