[Bug] Claude Code ignores user instructions in CLAUDE.md for code style and approach preferences

Bug Description
CC continuing to use over-terse, bordering on incomprehensible, English (in responses, code comments and Markdown files) but now, as an added bonus, bypassing my instructions to use the elements-of-style skill, which is in my global CLAUDE.md. An extra EXTRA bonus is it's also bypassing instructions about which code-writing approach to use and is trying to skip using subagent-driven. So we've just spent 5 minutes writing a hook to try to prevent that. Which is spending time writing code fixing problems with Claude Code itself, not doing the actual work I need to get done.
