[BUG] Resume command displays items from other users' accounts that disappear upon navigation

What's Wrong?

Resume command outputs show items that I haven't created - they may belong to another, unrelated, user.

After navigating to each of these items, an update occurs: claude-code returns to its initial state, and the suspected item is removed from the list when I try to resume again, but only that item. When I attempt to resume the next one, the same thing happens.

In the screenshot, first two items do not belong to my account.

What Should Happen?

claude-code should never display conversations from different accounts, even if it's just the title
