[Bug] Session survey interrupts active workflows with single-keypress submission hijacking

Bug Description
Issue: The "how's Claude doing?" session-quality survey interrupts active Claude Code workflows. When Claude sends numbered questions and I reply with numbered answers (e.g., "1. ... 2. ... 3. ..."), the survey sometimes appears mid-session and hijacks the same numeric input. Pressing "1" intending to answer my own question instead instantly submits it as a negative rating on Claude's performance — with no way to undo it. Impact: This happens multiple times per day and likely skews Anthropic's product quality metrics, since accidental single-keypress submissions don't reflect actual sentiment. Suggested fix: Don't make the survey a single-keypress submission — require confirmation before submitting, or use a UI cue clearly distinct from normal numbered replies.
