[Bug] Increased error rate with repeated mistakes in single interactions

Bug Description
you seem to be making more and more mistakes and then recognizing them while in the same interaction, or after i give some push back. I don;t remember you being this prone to mistakes. and also the are repeated and you keep highlighting how many times yiou have made the same mistake
