[Bug] Code review cycle creates cascading defects instead of reducing them

Bug Description
Pretty much every time, when the code is reviewed by "/code-review" there are 10-15 findings, the claude code fixes them, then next review finds even more, then again and again, it's like endless cycle, I just can't get to a state with reducing number of defects. Every fix generates more defects, unbelivaably annoying, tiring and exhausting.
