[Bug] Model fabricates user messages in conversation history with "human:" vs "user:" mismatch in JSONL logs

Bug Description
fabrication of turn entries. 'human:' is NOT 'user:' in the jsonl session logs. The model is confidently stating that I provided comments or guidance, that I DID NOT MAKE. Complete fabrication by the model and a significant safety issue in my opinion.
