[Bug] Opus 5 (1M context) repeatedly auto-downgrades to Opus 4.8 on benign messages due to overly strict safeguard

Bug Description
Opus 5 (1M context) keeps getting flagged and auto-switched to Opus 4.8
even on completely innocuous messages. Example: I asked only "am I on
model 5 with 1M context?" — a plain confirmation question, no code, no
sensitive request — and it still flagged and downgraded.
The trigger appears to be conversation context/history rather than the
current message, so re-selecting Opus 5 immediately gets flagged again,
creating a loop where I cannot stay on Opus 5 for the rest of the session.
Request: tune the safeguard so it doesn't repeatedly downgrade on benign
follow-up messages, and make it clearer to the user why it triggered.
