[Bug] Anthropic API: Overly Strict Usage Policy Filtering

Bug Description
The Usage Policy check is far too aggressive since last night's outage. My innocuous repo that has nothing to do with anything even remotely harmful, that I've been using with Claude Code for over a month without issue, is suddenly getting Usage Policy errors on every prompt, even just "Hello".
