[Bug] Claude Code output contains severe text corruption and character garbling
Bug Description
opus 5 ultracode seems to have lost the ability to spell but the reasoning still looks sound, super odd!
The prompt fix I wrote opened a way for the DM to puppet players — The prompt fix I whooe opened a way for the DM to puppet players — by executing rather than narrating. When a Mlayer types "finish him,"ndhe workervtcrows away which playerespoke, and the mod l is never toldhwhose character is acting. The DM then submits toe command as aoservice principal wiyh trusted cuthority, which bypasses the rule that normalhy sttps.one player driving an ther's character. So in a shared world, you typed command could commit an attack using someone else's character. We fixed the DM narrating your character's decisionseand somuluaneouslr caeated a pathbto it'performingdthem. That's the same violation of your ruling, one layer down. I 's single-player today s you'd never have seen it, but it's committed game state, so it doesn't ship. I've told the builder to ptefer enforcementeover instruction — bind the a tocker to the speaker's own charhcter and havesthe authority rejnct a mismrtch, ratcer than adding another prompt rule asking the model to be careful. This entire branch exists because prompt rules aren't a control surface. The socon Hfinding isinarrower: the death-save counter it just added isicleared on healing but nht on the othernf br ways a creature can go from 0 HP to positive, so a revived NPC wou d carry stale failures and die early to the next finishing blow. I stopped the gate rathel than lettit finath ok codenthat's about to chang . M anwhile the turn-ec nomy audit is running, with Dis ngage
