[Bug] Reduced Intelligence: Premature Context Compaction and Instruction Misinterpretation

Bug Description
reduced intelligence. not following instructions, doing the opposite of whats being asked. compacting conversation without context window being full (compacting after around 2000 lines) - premature compaction lol. Claude Code team, these issues have been on repeat for the last 2 months. the 99 percent uptime metric is completely false. its more like 78%. This reduced intelligence has been a recurring theme along with instructions and context related issues. along with many others along the way. no bug takes 2 months to resolve. specially when users are paying $200 a month and there are significant periods when the service is not running at 100%. in fact when theres reduced intelligence it actually makes life more difficult because then you have to fix all the issues that were created at claude code speed.
