[Bug] CLI doesn't distinguish between insufficient credits and monthly usage limit exceeded

Bug Description
When a model switch is blocked by the configured monthly extra-usage limit, the CLI says 'top up usage credits' — but the actual blocker (the spending cap) is never mentioned and is only visible in the claude.ai web UI. The CLI should distinguish 'no credits' from 'usage limit reached' and say which one applies.
