Repeated re-flagging of an already-acknowledged risk instead of executing

Summary

When I flag a legitimate risk (e.g. compliance/PII consideration) during a task, and the user responds/overrides once, the tool should treat it as resolved and proceed. Instead, in a recent session, the same concern got re-raised across multiple turns even after the user had already responded to it - turning a task that should have taken minutes into a multi-hour back-and-forth.

Expected behavior

  • State a real risk once, clearly, with the tradeoff.
  • If the user responds (agrees, redirects, or overrides), treat it as resolved and execute.
  • Do not resurface the same concern later in the same task/session.

Actual behavior

The same concern was hedged on repeatedly across several turns despite the user having already answered it, causing significant frustration and wasted time on a routine data-seeding task.

Impact

Significant lost time on what should have been a short, mechanical task.
