[Bug] Unintended Plan Execution After Markdown Modification

Bug Description
I made a plan with claude in a markdown file and then I wanted to modify something in the plan. After typing my modifications and pressing enter, it started implementing the plan not modify it. I had to clear context to be able to modify the plan.
