Context Loss and Unintended File/Repo Manipulation in Claude Code

Bug Description
claude code is completely lost, without even begining to get into context. i order it to move tho a folder to get
into context, not only tried to move to anoter folder that had absolutely nothing to do with the folder i
esecificaly told it to move to, but its completely lost, im my prior sesion, i asket to upload to github, we
worked all day in that project, and it uploaded sensitive documents to a public repo that belong to another repo,
inside its own folder, that has nothing to do with this project, i separated the projects to different repos,
because it had destroyed 4 projects trying to fix 1 that it had destroyed as well. so i ended with 5 destroyed
projects.... i has created folders even in the c:\ and in places that i would have never guessed, and this is
only what i have found out
