Bug Description
I get another "Claude Code is unable to respond to this request, which appears to violate our Usage Policy" because I'm doing a security audit on a local repo for a customer. It happens a lot, sometimes for no reason at all. This is a really big issue because it makes Claude Code unusable for perfectly normal work.
