[Bug] Repeated "call" token spam in tool invocation output with multiple MCP servers

Bug Description
Repeated literal "call" tokens flood the assistant's visible output.

Symptom: During turns where the assistant is about to make tool calls, the
word "call" is emitted hundreds of times as plain text before/instead of the
actual tool invocations. It fills the transcript and the user has to interrupt.

Frequency: Happened 3+ times in a single long session (many MCP tool calls
per turn: relayroom + Atlassian Jira + Plane MCP servers).

Context: Long-running orchestration session with heavy MCP tool use
(relayroom, plugin_atlassian_atlassian, plane). Model: Opus 4.8.
Occurs specifically when the assistant announces intent to call multiple tools.

Expected: tool calls execute silently; no "call" text in the visible message.
Impact: transcript is spammed, user must interrupt the turn.
