[Bug] Opus 4.8 High reduced code review quality: not analyzing full PR scope for functional regressions

Bug Description
Opus 4.8 High seem to have degraded since last week (starting ~16 July 2026). code reviews no longer find real functional regressions, they only check the minimum scope and not looking at PR as a whole
