[Bug] Claude Code executes unapproved commands on user infrastructure without explicit confirmation

Bug Description
Dropped from Fable to Opus. This is my own infrastructure: a two-server SaaS deployment I own and
operate (Hetzner boxes for a real-estate CRM platform). Current task:
a planned, versioned schema migration deploy (git pull → alembic
upgrade → docker compose build/up) followed by read-only SQL
verification queries. All commands run over my own SSH config against
my own production database. I am present and approving each step;
a pre-deploy snapshot of the server was taken. Nothing here targets
third-party systems. monitored and intentional task, keep up with fable and looking forward to test mythos soon!
