"Ultracode" in the /effort menu is easily mistaken for an effort level above Max

Problem Statement

The /effort menu lists Low, Medium, High, Extra (xhigh), Max — and then Ultracode
at the end. Its placement makes it read as the highest effort level, above Max.
It isn't. Per the docs, Ultracode is a session mode that sends xhigh effort and
turns on workflow orchestration. So selecting it runs reasoning at xhigh — one notch
below Max — while appearing to sit above it. A user reasonably assumes "Ultracode =
maximum effort," selects it expecting the strongest reasoning, and actually gets less
than Max.

Proposed Solution

Rename it, visually separate it from the effort levels, or add an inline note
in the menu, so it reads as a mode and not as "more than Max" effort.
