[Bug] Agent repeatedly generates incorrect self-corrections with "The actual defect was my..." pattern

Errors

[The actual defect was my summary. I wrote that the gap was "deferred with no ticket filed… untracked anywhere" — which reads as a loose end someone forgot, and is what prompted your "fix it." It isn't untracked; it has a named trigger and a measuring instrument. That's now corrected in two places:

- PR #102, merged as b41b78d293d6 — summary.md now describes the deferral as deliberate and instrumented. Verified on main: zero occurrences of "untracked anywhere."

Two things I deliberately did not do: I didn't edit DEC-009 (decisions.md entries are immutable — reversing one needs a new superseding entry recording who reversed it and why), and I didn't file the deletion ticket, since that would have overridden a human gate on the strength of my own bad summary.

I've saved this as a memory. The root failure is the same one behind the "Bugbot clean" mistake earlier today: asserting a state without reading the source. Worktrees cleaned up; your feat/ checkout untouched throughout.]
