[Bug] File Picker Fails to List Complete Directory Contents

Bug Description
The file picker doesn't work and only shows a handful of files at the top-level. This makes claude code completely useless, this has been happening for days, I think since 1.0.112. Claude code is effectively completely down right now, as removing a fuzzy file picker renders the entire tool pretty much unusable.
