Plan panel comments not relayed to Claude

Description

The Plan-mode UI's "Select any text to leave a comment for Claude" feature (Plan panel) does not appear to relay left comments into Claude's context automatically. Comments left via that panel are not surfaced to the model in the conversation — the only way to get them through is for the user to manually screenshot or paste each comment as a separate chat message.

Expected behavior

Comments left on plan text via the Plan panel's inline comment feature should be automatically relayed into Claude's context (e.g., injected as part of the next turn), the same way other plan-mode state is synced.

Actual behavior

Comments added through the Plan panel are invisible to Claude. No tool or context injection surfaces them. The user has to manually screenshot/paste each one.
