[Bug] Agent fails to implement agreed specifications and generates new designs instead of following mockups
Bug Description
I've been working with this agent to build out a UI. We went through many many rounds of mockup generation and implementation approval, nailing down the exact features functionality and looks, and when it went to go implement it, it first decided that it was only going to build 1/4 of the agreed upon work and happily told me it was finished and then only "realized" it hadn't done the agreed upon work after I pushed back, then it spent another 2 hours burning tokens to implement what we had agreed to, only for me to catch that it hadn't actually done that, it had hand-wavingly described the requirements to an agent and then had it build something entirely new from scratch instead of using the mocks we had used and it had agreed to build with. This has burn days of work and hundreds of thousands of tokens on this just fundamentally not working. Opus used to be really quite good, this is an abject failure.
