[Bug] Claude Code: Edit Operations Failing Despite Enabled Permissions

Bug Description
Claude no longer performs edits despite saying that it has. I've checked tool permissions. They are vanilla defaults, which means edits are fully enabled. I've checked permissions masks. They are all at least 755. Since Claude runs as my user, that means its access is unencumbered with rwx.
