[Bug] Opus Reasoning Degradation: Incorrect Assumptions and Poor Comprehension

Bug Description
Opus has started to feel like it's just not thinking at _all_. It's making the most ridiculous assumptions about the code, the instructions, and even when I said "button should always show the color regardless of preference" it assumed this applies to the text as well (when it was clarified it does not in a prior prompt). The EXACT same prompt to Codex determined the root cause, reasoned correctly, when challenged it responded after reading and understanding instead of blinding saying "You're absolutely right" and then proceeded to fixing the issue.
