[Bug] Claude Code: Incorrect Layout Handling with Persistent Misplacement of Elements

Bug Description
claude is failing to follow instructions... in a simple html page build with like 6 elements following a template , buttons were placed in the wrong spots.. i ask specifically that two buttons had to be on the bottom and not vertical on the right.. claude instead of changing the code just written went in a tirade of patches and !important and forces to make the changes???? this has been consistent.. and when asked it says yah i messed up let me fix, then adds more patches and more !important.. every request it guesses without looking at the code first even when the instructions are explicit and clear. regardless what prompts guides and instructions used, its like coding with a schizophrenic drunk.
