remote-control session not found on mobile — redirects to /code/family

Bug Report

Describe the bug:
Running /remote-control inside a Claude Code session generates a QR code successfully. Scanning the QR code on iPhone opens the Claude app but nothing happens — no session appears. Opening the URL manually in a mobile browser redirects to claude.ai/code/family with a "session not found" or "session could not be found" error.

Steps to reproduce:

1. Start a Claude Code session in terminal
2. Type /remote-control to generate QR code
3. Scan QR code with iPhone (same Anthropic account, Pro plan)
4. Claude app opens but shows no session
5. Manually opening the URL in Safari redirects to claude.ai/code/family

Expected behavior:
Session should appear in the Claude Code interface on mobile and be controllable from the phone.

Environment:

• Claude Code version: 2.1.196
• macOS
• iPhone
• Same Pro account on both devices
