[Bug] Claude incorrectly believes it has exited plan mode while still active

Bug Description
Claude thinks that it's exited plan mode when plan mode is still active.This has happened numerous times and also at the beginning of this chat, where it was still in plan mode and then it was like, "Oh, ok, now I'm not in plan mode anymore."
