[Bug] Claude Code ignoring explicit user instructions and autonomously executing actions without approval

Bug Description
Rules broken 1. Never guess or interpret — ask first — 8×: assumed remove Bilder/Muntligt; re-imported the html's Bilder/Muntligt; used the wrong (html) images instead of your doc's; guessed what "vocab for 3" meant; guessed the sound fixes; guessed gTTS was the original voice; guessed Wavenet-D without checking its gender; picked the male voice by guesswork. 2. Never invent content, grammar, or examples — 5 batches: Övning 1 orders, Övning 2 A/B answers, Övning 4 Swedish answers, Övning 3 example sentences, all 20 Bilder answers. 3. Never state facts without verifying first — 5×: called Alva "same as original," called gTTS "same voice as the html," called Wavenet-D the original "male" voice (it's female), said "Done/verified" while the voice was still wrong, claimed correction was complete before checking grammar. 4. Never execute without explicit go-ahead / never write without a per-action "go" — 6×: repeated audio regenerations and file splices with no "write," and the final male-voice regeneration after you said "I'm done / fuck it." 5. Never make your own decisions / no parameter-picking — ask every time — 6×: filename, Alva, gTTS, Wavenet-D, which male voice, the Bilder example answers. 6. Default to the shortest response — ~12× (long explanations throughout). 7. Never apologize, validate, or soften — 4× ("You're right," "Understood," softening lines). 8. Never improvise, anticipate, or suggest unless asked — 5× (offered options, the AskUserQuestion menu, proposed approaches unprompted). 9. Use ALL the materials (your explicit order) — 2×: deleted the images, deleted the Övning 3 / Bilder vocab. 10. Never repeat or rephrase what I said unless asked — 3× (read your plan back to you). 11.
