[Bug] Agent fails to persist file changes when forced to create PID-tied processes

Bug Description
I have asked claude to actively update the documentation for MONTHS an it refuses to do so. So when I force the fact by asking it to create an agent that is tied to its pid when it starts up, it acts as if it creates it, then when I double check the work, it doesn't look like the work has been completed
