[Bug] MCP initialization error on startup and /doctor command shows non-existent tools

Question

**Bug Description**
I keep getting 1 setup issue: MCP · /doctor when loading claude intermittently, but when i run /doctor it often shows me an error on an mcp I dont have which is quite worrying.
