[Bug] AskUserQuestion dialog persists after user response; session hangs with incorrect subagent attribution

Bug Description
The "Waiting for team lead approval" is shown (for AskUserQuestion) and doesn't go away. I did see the question and answered it but the dialog stays up. The request was also issued by a subagent that isnt active anymore - i think it was actually issued by the main session but it was attributed to a subagent. The session is hanging now and doesn't want to proceed. I had to cancel the conversation, which resulted in "USer declined to answer questions" although I DID answer.
