[BUG] Insane ask

What's Wrong?

Unexpectedly, Claude showeed this strange. popup saying it was adding +88,000 lines to a single file. This is unexpected behavior. On 4.8 Opus and macos darwin 25.5.0

What Should Happen?

Claude should NOT do this.
