What's Wrong?
The MCP approval dialog for notion-create-pages and notion-update-page does not appear in the desktop app. The tool call immediately fails with the error below. This was working fine until midday on June 15, 2026 (JST).
What Should Happen?
An approval dialog should appear in the desktop app so the user can click "Allow".

Streamable HTTP error: Error POSTing to endpoint: MCP tool call requires approval
Steps to Reproduce
- Open Claude Code desktop app (latest version)
- Start a session with Notion MCP configured
- Ask Claude to call notion-create-pages
- No approval dialog appears — the call immediately fails with the error above

Additional Information
- org_max_permission is set to "ask" for notion-create-pages
- PC restart, app restart, and MCP toggle did not resolve the issue
- Google Drive MCP (read-only) still works fine
- The issue started around midday on June 15, 2026 JST
