Title: [BUG] Cowork bash sandbox stuck "Workspace still starting" indefinitely on macOS

Description:

The bash sandbox (mcp__workspace__bash) never becomes ready in a Cowork session on macOS. Every call returns "Workspace still starting" regardless of how long I wait (tested 10+ minutes, 15+ retries).
Steps to reproduce:

Open Claude desktop app (Cowork mode) on macOS
Ask Claude to run any bash command
Observe: every attempt returns "Workspace still starting"

Expected: Sandbox boots within 10–30 seconds

Actual: Never boots; stuck indefinitely for the entire session
Workaround: Starting a new conversation sometimes resolves it
