[Bug] Anthropic API Error: Inconsistent false positive policy violations on legitimate addiction-recovery educational content

Bug Description
We publish addictionhelp.com, an addiction-recovery education site (clinical/ educational content on alcohol, drug, and behavioral addictions). On June 11–12, 2026, Claude Code began returning "appears to violate our Usage Policy" on routine editorial work: rewriting pages about alcoholic liver disease, Wernicke-Korsakoff syndrome, naltrexone, GLP-1 medications for alcohol use disorder, and similar clinical topics. Identical parallel requests passed and failed inconsistently (9 of 11 page rewrites passed, 2 blocked). Eventually plain conversational messages ("What do I do next?") were blocked in that session.
