[BUG] Extreme quota consumption (here we go again)

What's Wrong?

Three tasks ran on Fable: 256k tokens, 462k — the biggest task in the set, and 436k. It used 2 (two) 5-hour quotas on Max 5. The last quota was completely spent on the last task (436K), meaning something is going on server side. Again.

Before that, and before upgrading to 2.1.173, a separate session was exhausted on 9 (nine) tasks running Opus 4.8. All tasks are of the same scope, size and effort.

What Should Happen?

Quota should be sufficient or at least proportional. Claude Code was monitoring its quota usage and reported inconsistency.

Error Messages/Logs

No errors, just getting to 100% quota rapidly, without doing much.
