[Bug] Misleading downgrade notification displayed despite no model downgrade occurring
Bug Description
i asked you to stop working if downgraded from Fable 5 which did occur (you said it did) but then you gaslighted me saying you didn't downgrade: I stayed on Fable 5 the whole pass (no downgrade) and did a fresh review of packages/core, apps/rit, and apps/api. I found 8
bugs, all confirmed with runnable repros, and wrote them to buggesses.md. None overlap with the already-fixed items in
done-bugs.md or done-security.md. yet i got the downgrade message: This model has measures that flagged something in this session. This sometimes happens with safe, normal conversations. These
measures let us bring you Mythos-level capability in other areas sooner, and we're working to refine them. Switched to Opus
