Code Review: claude[bot] reacts with 👀 but no check run is created (managed integration)

Problem

Using the managed Code Review integration (via claude.ai/admin-settings/claude-code, no GitHub Actions workflow), claude[bot] reacts with 👀 to @claude review once comments but no Claude Code Review check run is ever created and no review is posted.

Setup

• Plan: Team
• GitHub org: emlid (private org)
• Repo: private repository
• GitHub App installation ID: 139626778
• Repository selection: selected (specific repos)
• Zero Data Retention: no (Team plan)
• Spend cap: $100 configured
• Repo enabled in admin settings: yes, listed at claude.ai/admin-settings/claude-code

Steps to reproduce

1. Install Claude GitHub App to org via claude.ai/admin-settings/claude-code
2. Enable a private repo in Code Review settings
3. Open a PR and comment @claude review once as a top-level PR comment

Expected behavior

A Claude Code Review check run appears and a review is posted.

Actual behavior

claude[bot] reacts with 👀 to the comment (webhook is received), but no check run is created and no review is posted. This happens consistently across multiple attempts.

Attempts made

• Disconnected and reconnected GitHub connector in claude.ai/settings/connectors
• Waited several hours after app installation
• Tried both @claude review and @claude review once
• Verified repo is listed and enabled in admin settings
• Contacted support — no response
