[FEATURE] Show remote SSH host indicator in Claude Desktop app

Problem Statement

When using Claude Code inside the Claude Desktop app while connected to a remote server, there's no visual indicator showing which host you're connected to. VS Code shows this in the bottom-left status bar. It would be helpful to have a similar persistent indicator in the Desktop app UI.
