[BUG] advisor_tool_results

What's Wrong?

I'm running a session with 2.1.162 in CC. I've tried asking /btw and get this error:

/btw why ********?
/btw why ********?

API Error: 400 messages.5.content.0: unexpected tool_use_id found in advisor_tool_result blocks: **********. Each advisor_tool_result block must have a corresponding server_tool_use block before it.

What Should Happen?

just in iterm2 terminal, when i use "/btw" to ask a question i get the error.

Steps to Reproduce

I've done three /btw calls and it happens every time. I'm also in ultracode effort mode for the session
