[Bug] Anthropic API Error: False positive Usage Policy violation during mid-session execution

Bug Description
Claude code repeatedly errors out for me with the message:API Error: Claude Code is unable to respond to this request, which appears to violate our Usage Policy. It is clearly a "false positive". It happens e.g. mid-session while claude is doing something. If I then ask it to save I get the same message. This has happened in four sessions today.
